MEDICAL CERTIFICATE & HONOR STATEMENT

The following documentation must be completed by the runner and uploaded to their MyUTMB Profile before February 15, 2026, for review and confirmation by the event organization. No runner will be allowed to participate in the event without the corresponding approval, with no exceptions.
